Resume. A copy of the applicant's current resume.
Authorization Letter. Authorization by the by Ministério de Tutela and opinion issued by the Ministry for Public Administration, Labor and Social Security (MAPESS-Ministero da Administração Publica, Emprego e Segurança Social).

Visa application form. Two fully completed and signed application forms.
Original passport. The passport must be valid at least 6 months after the intended date of departure from the territory of the destination country and has at least 2 blank visa pages.
Passport-type photograph. Include 2 passport style photos, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a charge associated with this service.
Proof of status. A copy of immigration status in the Unite States such as a resident card, work, or study permit. VisaHQ cannot assist US B1/B2 visa holders at this time.
Itinerary. A copy of round trip tickets or detailed itinerary for the duration of the trip. The applicant's name must be on itinerary.
Bank statement (per day). A copy of the applicant's most recent monthly bank statement showing at least $200 per day. The statement must clearly show the applicant's name as the account holder, the balances of the accounts, and the date of the statement.
Employment Invitation letter. A letter from the sponsoring company in Angola. The letter should be printed on company letterhead stationery, addressed to the Consular Mission Chief, and signed by a senior manager. The letter must adhere to the following guidelines: briefly introduce the applicant; state the nature of the position being offered, salary structure, and duration of the contract.
Certificate of Employment. Work certificate. Please provide a copy of the applicant's employment contract signed by the representative of the company or organization employing, hiring or sponsoring the trip, stating specifically the job to be performed by applicant, the duration of the contract, as well as the fact that employer takes full financial responsibility for the applicant. This document needs to be authenticated by a Public Notary and should be addressed to The Consulate General in Houston.
Personal letter. Statement authenticated by a Public Notary indicating that applicant agrees to abide by Angolan laws.
Police clearance. Notarized Police Clearance Certificate authenticated by the Department of State stating that applicant has no pending criminal proceedings.
Vaccination certificate. International Vaccination Certificate or Medical Certificate of Health.
