Applicants under 18 years of age are considered minors and cannot sign their visa application forms. Instead, the forms must be signed by one or both parents
(signatures on this form do not need to be notarized).
• Polio vaccination.
For applicants between the ages of 3 months and 6 years, a certificate of vaccination against Polio is required. If the child cannot be inoculated, please submit a notarized letter from the child's physician.
