Indian visa application form. Completed application form for India will be prepared within 5 business hours of order submission, and emailed to you to download, print and sign. IMPORTANT: Please note that ALL 3 PAGES of your application require your original signature! Also please note that each page of your application must be printed SINGLE-SIDED only. Applications printed/signed double-sided will be rejected.
Original, signed Democratic Republic of the Congo passport with at least 6 months of remaining validity.
Passport Photo: 1 Include a passport style photo, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a surcharge associated with this service.
Proof of status. Copy of Green Card (both sides) or other proof of legal status in the US (such as copy of I-20, US visa, H1B approval notice, etc. VisaHQ cannot assist US B1/B2 visa holders at this time.)
Address. Address in Democratic Republic of the Congo. If the traveler no longer maintains a residence in Democratic Republic of the Congo, they may provide their most recent residential address or the address of a relative.
Driver's license. Copy of driver's license or state issued ID, or ORIGINAL major utility bill (Water, Gas, Electric, Sewage) for the most recent month, showing the applicant's name and current address. The address must not contain a PO Box. Address MUST match home address in your applicant profile.
Applicant must not wear glasses in their photographs.Passport MUST contain at least two blank visa pages in order for the visa to be issued.
Former citizens of India
Former Indian Nationals that have been Naturalized must provide copy of stamped Renunciation Form or Deemed Surrender Certificate in order to apply.

• Completed and signed Renunciation Form or Deemed Surrender Certificate.
• Copy of Naturalization certificate.
• Either the original Indian passport if you have it or a copy of your indian passport if you do not have it.
• For a child 5 years and older, signatures of the child and BOTH PARENTS are required.
• For child less than 5 years of age thumb impression of the minor child and signatures of BOTH PARENTS are mandatory.
Additional required documents for applicants who have undergone a name change
• Marriage Certificate: A copy of the Marriage Certificate if the applicant's name differs between the documents provided
Additional required documents for minors (under 18)
• Application form.
The minor must sign or print his or her name in the signature box under the photograph on page one. Minors that are too young to print their name should have their thumbprint stamped into the box in blue or black ink. The second page of the application form must be signed by both parents
and the signatures must be NOTARIZED
• Birth certificate.
A photocopy of the applicant's birth certificate. The birth certificate must be the "long form" certificate which shows the names and nationalities of the applicant's parents. Applicants born outside of the US who cannot obtain a copy of their birth certificate may submit a copy of an official document issued by Government authorities, provided it shows their personal particulars like name, date of birth, parents names, and nationality. Foreign language documents must be accompanied by a notarized English translation.
• Parental Authorization.
A Parental Authorization for Minors form from the Indian Consulate, signed by both parents. This form is available here
• Additional Particulars Form.
The Additional Particulars Form must be signed by both parents.
• Parents' passport copies. NOTARIZED
copy of the information pages of both parents' signed passports.
• Parents' driver's licenses. NOTARIZED
copy of both parents' driver's licenses.
• For Single Parents.
A copy of the court documents proving legal and physical custody of the minor. If the spouse is deceased, please also submit a copy of the parent's death certificate.
Persons with Occupation Related to Media
For journalists, writers, photographers, designers, and those who work with media; regardless of purpose in India, must apply for a Journalist Visa.
Applicants of Sri Lankan origin
Any applicant, regardless of their nationality, of Sri Lankan origin must fill out an additional application form. The form can be found here
Visa validity alert
Applicants are strongly recommended to apply for the shortest duration necessary. The Embassy may issue a lesser validity visa at their discretion, regardless of the fee that is paid.
Applicants of Bangladeshi Origin
Any applicant, regardless of their nationality, of Bangladeshi origin must fill out an additional application form, IN DUPLICATE
, and must provide 2 ADDITIONAL passport photos

Persons with Occupation Related to Religion
Any applicant with an occupation related to religion must also include a completed and signed Religious Undertaking Letter. The form can be found here
. Such applications will extend processing times and may receive a visa with shorter validity.
According to Indian Consulate's regulations, the passport will be mailed DIRECTLY to the traveler once their visa is issued. The passport will be shipped via FedEx or UPS and will only be sent to your RESIDENTIAL ADDRESS. Therefore it is mandatory that the home address listed on your application is accurate and matches your driver’s license or a current utility bill.
The return delivery fee of $15 has been added as part of the standard consular fee. Please note this fee may be higher for residents in certain parts of Alaska and Hawaii where standard FedEx service is not available.
