The application for any passport service must be completed online at the U.S. Department of State website. Upon completion of the application form tick the 'I have read and acknowledge the steps contained above.' checkbox and proceed to downloading the PDF application form by clicking the 'Create Form' button. Make sure that your application form has the barcode on it before proceeding.

Don't submit this form. A missing barcode means the application wasn't generated through the State Department system — go back and use the 'Create Form' button.
You can prove your identity with one of the following documents.
A valid or expired, undamaged passport book or card previously issued to you.Counts as damaged: water damage, mold, stains, significant tears, unofficial markings, or missing pages. Folded pages or small bends are fine.
The original certificate issued at the time of your naturalization.
The original certificate issued to you as evidence of U.S. citizenship.

Bring a certified copy — not a photocopy.
Other situations:
· If your name changed during naturalization, the Certificate of Naturalization itself is sufficient evidence.
· For an informal name change without a court decree or marriage ("custom and usage"), submit Form DS-60 with evidence of using the new name publicly and exclusively for at least 5 years (3+ public documents, or 2 affidavits from people who've known you under both names).
You can still apply — you'll need to combine signature documents with someone willing to vouch for you.
